About This Tool
If you send out a flat PDF and ask people to fill it in, you get back photographs of paper at various angles, handwriting you cannot read, and three people who typed their answers into the email instead.
Adding real form fields fixes that. Drag a text field over each blank, a checkbox onto each tick box, a dropdown where the answer should come from a fixed list. Each field gets a name, so the data has structure if you later export it, plus optional default values and a required flag.
The result is a standard AcroForm PDF. It opens and fills correctly in Acrobat, Preview, browser viewers and mobile readers - you are not asking recipients to install anything.
